Job description:

  • Bradford, VT
  • Randolph, VT
  • Springfield, VT
  • White River Junction, VT
  • Lebanon, NH
• Your role as an RN case manager:
  • Patient care: Screen, evaluate, and treat patients while coordinating with interdisciplinary team members.
  • Care planning:
    • Develop and periodically review care plans in collaboration with patients, their families, and physicians.
    • Ensure effective communication of care plans through case conferences.
  • Ongoing assessment: Monitor clinical, functional, and emotional status to identify progress and challenges.
  • Education & support: Teach patients and caregivers self-care techniques while providing essential health guidance.
• Minimum qualifications:
  • Licensed registered nurse in Vermont and New Hampshire or hold an unencumbered compact license.
  • Graduate of an accredited school of nursing.
  • 2 years of experience in home health.
